All students need to have the following forms on file with the nurse. These forms must be updated yearly.
Late Bus
PCMS offers a late bus for students who need to stay after school on Tuesdays, Wednesdays, and Thursdays. Any child may ride the bus as long as they have a signed permission slip on file. Click this link for a permission slip. Buses leave school at approximately 4:00 p.m. The three late bus routes have been established with 'cluster stops" throughout town. Permission over the phone will NOT be accepted. Please make sure he/she knows the route color and stop location. It will be up to your child to let the after-school supervising staff member know where he/she needs to get off.
